Individual Advance Care Planning advice and support
This can be one-on-one with the person who wants to undertake ACP or I can meet with the person and their family members and help them decide who should be appointed as Enduring Guardian and how they can ensure that the person's wishes in their Advance Care Directive are respected. This would include an explanation of the law in the person's State or Territory.
Attending medical appointments with older people who need someone else to ask questions and hear the responses.
Representing patients and/or their families in interactions with doctors, hospitals, residential aged care facilities (previously nursing homes) and/or community care agencies; this can be in person, by phone or by written communication.
Presentations and Information Sessions
For nurses, doctors, allied health staff, residential and/or community care staff, residents & their families, older people's groups, church groups - (anyone who wants one) - on Advance Care Planning; end-of-life issues (including what is/is not euthanasia; PEG Feeding; the Right to Adequate Pain Relief in Terminal Illness); the Benefits of an Ageing Population; Using Telehealth to improve the health of older people and people with disabilities; Futures Planning for older carers of adults with disability.
Staff Training Workshops
For nurses, doctors, allied health staff, residential and/or community care staff. Mainly on the issues listed above but also including Capacity Assessment; State-specific legislation around who has the legal authority to consent to withdrawing/ withholding life-sustaining treatment from an adult who lacks capacity; how to start the ACP conversation with patients/residents.
Research and Evaluation of age-related topics
In particular end-of-life and advance care planning issues, as well evaluation of healthy ageing initiatives, telehealth, residential care and community care.
Expert Witness Service for solicitors
In particular around disputes relating to people in residential care (e.g., between family members about the person's care/ treatment or between the family and health-care providers).
